FAQ's to Help Answer Your Questions

HOW CAN i OR mY CHILD BEGIN RECEIVING SERVICES AT THE ACHIEVE CENTER?
​All it takes is a phone call to the Achieve Center office and we will assist you. Call us today at 715-845-4900.
iS A REFERRAL FROM A PHYSICIAN REQUIRED?
Unless required by your health insurance policy, a physician’s referral is not required for treatment to begin. However, for speech and language therapy or assessment through the Space Clinic, a physician’s referral is required to establish medical necessity for medical insurance coverage.
wHAT FORMS WILL I NEED TO COMPLETE?
 Yes, there are forms that you will need to complete. The Personal History form is detailed and does require approximately 30 minutes to complete it. There is also a General Information form which provides your address, phone number, insurance company, and other similar information, and Release of Information forms for your signature so medical records or school records can be obtain for continuity of care.

wHAT CAN I EXPECT AT MY FIRST APPOINTMENT?
​At the first appointment, the Client Rights and the Clinic Fee Policy will be verbally reviewed with you and you will receive a printed copy. You will also be asked to sign a Consent for Treatment form as required by Wisconsin law. During the session with the clinician, you will be asked questions about the concerns you have and the history relevant to these concerns. A Treatment Plan will be developed with you as a guide for treatment. This Treatment Plan is reviewed at each session and can be modified to reflect progress and to add goals for treatment.
wHAT ARE MY PAYMENT OPTIONS?
​Most forms of health insurance and Badger Care are accepted. A sliding fee schedule is also available and is based on federal guidelines. To meet these guidelines, an application must be completed. Copies of W2s, most recent pay stubs, and statements from all other sources of income for all individuals residing in your household must also accompany the application. Your fee for services is then determined by the established fee on the schedule for your household income bracket.

A payment plan can also be arranged for those with high insurance deductibles or for those who must pay privately and choose not to apply for a sliding fee. Please contact the Achieve Center Billing and Insurance Specialist, who will be happy to assist you with all of your billing and payment questions and arrangements.
What safety precautions Is the Achieve Center Taking during Covid-19?

VIRUS INFECTION CONTROL POLICY
 
In an effort to continue to prevent the threat of Achieve Center employees, patients, and families being exposed to viral infections, the following policies have been established:

  1. Masks must be worn at all times while in the building. Except:
    1. An employee is alone in an office. However, upon entry of another person, the employee must wear a mask
    2. While wearing a face shield and providing direct treatment for a communication disorder to a child. However, upon completion of direct treatment and during discussion with parents/caregivers, the employee must wear a mask
    3. Children ages 36 months and under
    4. While a child is receiving direct treatment for a communication disorder or a child receiving direct behavioral treatment, who has a communication disorder, and whose speech would be hampered by a mask. However, upon completion of direct treatment and before leaving the treatment room, the child must put on a mask.
    5. If wearing a mask is an emotional issue for the child, working towards the child wearing a mask needs to be a treatment goal.
    6. Employees are required to wear a clean mask daily. Cloth masks, after wearing one day, should be washed before wearing the next day. If an employee prefers paper masks, a new one should be worn each day
  2. All patients, families, and other visitors will have their temperatures taken upon entry to the building. Employees, at the beginning of their morning schedules and at the beginning of their afternoon work schedules, will have their temperatures taken by an administrative staff member who will maintain a daily completion list. Entry will be denied if an elevated temperature is recorded.      No Exceptions
  3. All patients, families, and visitors will wash their hands upon entry to the building. All employees will wash their hands upon entry to the building and clinical staff, prior to the start and after completion of a treatment session. All employees are reminded to wash their hands frequently throughout the day.        No Exceptions
  4. It is the responsibility of all employees to assist with maintaining a clean work environment.
    1. Administrative employees will sanitize the front office, reception and waiting area frequently each day especially those supplies and equipment utilized by the general public which will sanitized after each use.
    2. Clinical employees will sanitize their work areas, supplies, and equipment daily including treatment rooms and therapy supplies and tools which will be sanitized after each use.
  5. All employees are requested to avoid large gatherings of people, maintain a distance of greater than 6 feet from others, and to wear masks when in the company of others who are not members of their household. In the event an employee is in a situation where masks are not worn or a situation where there is not physical distancing whether masks are worn or not, the employee is asked to isolate 14 days prior to returning to work.                *Note: Caution is important and represents care and respect for co-workers.
  6. All employees are to immediately report any known exposure to Covid-19. The Achieve Center will immediately notify employees of any reports it receives related to patients receiving a diagnoses of Covid-19.
    1. If exposure occurs, the effected staff will be required to isolate for 14 days in their homes from the date of the exposure.
  7. If an employee records or reports an elevated temperature or reports possible other symptoms of a viral infection they must:
    1. Leave the Center
    2. Consult with their physician
    3. Obtain a Covid-19 test
    4. To return to work an employee must:
    5. Present a written confirmation of a negative Covid-19 test to Michael Sladky, Business Manager or Carol Wesley, Executive Director
    6. Not have an elevated temperature for greater than 48 hours   *Note: The decision for an employee to return to work is at the discretion of Michael Sladky, Business Manager or Carol Wesley, Executive Director and will be based upon their best judgment related to safety of others.
  8. All employees who are diagnosed with Covid-19 must report the diagnosis immediately to Michael Sladky, Business Manager or Carol Wesley, Executive Director so that others, who may have been exposed, can be notified and precautions taken.
  9. For employees who have been exposed but are not symptomatic of an infection and are isolating at home, every effort, if possible, will be made to provide them duties, which are in the scope of their job description, which will allow them to continue working from home.
  10.  The health and well being of employees, patients, and families is a priority of the Achieve Center. The current health crisis facing our community is serious and every effort will be made to insure a safe and secure environment for employees, patients, and families.
